Church Event Checklist/Request Form

This form will need to be filled out for EVERY event that takes place at Innovation. Please be sure to fill out every portion of the form. Thank You!
  • Date Format: MM slash DD slash YYYY
  • Describe as if you were inviting someone off the street that has never been there. Avoid phrases like "at the church" (say at Innovation) and any phrases that sound too "christianise"

  • Try to be mindful of starting an event at a time that people can make it after work. *We prefer all events to have a start time at 6:30pm, to keep it consistent with all Innovation events.
  • * We LOVE getting pics of what's going on at Innovation! Be sure to assign this task early so you are sure to have someone available!
  • *Note: is your responsibility to gather a team of volunteers for your event (down forget to get volunteers for set-up AND tear down * Any event with children 18 and under, the volunteers MUST be background checked.
  • To help guest services stay free for guests and stay uncluttered, we encourage you to have digital registrations and signups. Save PAPER signups for when we pass it around to the congregation during a service. (this is only for big events and should rarely happen)
  • * It is YOUR responsibility to find childcare workers. *PLEASE NOTE: There MUST be at least 1 adult and they MUST have a background check through the church.
    *PLEASE NOTE: All church events MUST HAVE graphics pre-approved/ and or be designed in house by our graphic designer.
  • *PLEASE NOTE: Announcements MUST be pre-approved to go into announcement video by senior pastor If its on the video announcements it MUST also be on the website as well.
  • *PLEASE NOTE: Announcements MUST be pre-approved by senior pastor
  • * PLEASE NOTE: MUST be pre-approved by senior pastor
  • PLEASE NOTE: This must be approved before you can purchase items for your event.
  • Please list items, vendor/supplier and cost for each item.
